What is checked
The page looks for a urlset and for loc text. A location must start with http:// or https:// and contain no spaces. Repeated locations are listed once for each extra copy. lastmod tags are counted, not checked against a calendar.
A sitemap index that points at other sitemap files is not expanded. Paste each urlset you want counted. The page does not download the file from your site.

Questions
Does a warning mean Google will reject the file?
Not by itself. A relative URL or a duplicate is a problem worth fixing. Google’s own report is the one that decides indexing.
Are URLs inside comments counted?
The scan looks for loc tags. Keep comments free of fake loc tags if you do not want them counted.
Can I check a live URL?
Paste the file. This page does not fetch it.
