How a heading is read
The page finds h1 through h6 tags and uses the text inside, with nested tags removed. A heading written as markdown, such as a line of hashes, is not a heading here. Paste the HTML.
One H1 is the usual shape. A skip means a level was jumped, which makes the outline harder to follow. The page does not rewrite the HTML.

Will a heading in a div with a class be found?
Only real h1–h6 elements. A styled div is not a heading.
What if the H1 is in a template?
